Powercom America, Inc. Policy
1.
 All items should be put in original package & Powercom doesn't respond in shipping damages without original package.
2.
 The related invoices should be faxed along with this form for all items, otherwise they will processed as Out Of Warranty and will bill parts, labor, plus S&H fee.
3.
 $19.95 S&H fee will be applicable for each item if purchased date was over 1 year but the item is still under warranty.
4.
 For any DOA (Dead On Arrival) item replacement, credit requisition, and resellable condition, please call the related Salesperson for details.
5.
